Often, clients choose to privately pay for sessions in order to maintain privacy and choice of treatment. I can provide a super bill for you to submit to insurance for potential reimbursement. If you do choose to submit your receipt to insurance for reimbursement, please note that I will be required to provide a diagnosis which will become a part of your permanent insurance record.
Comprehensive Diagnostic Evaluation
Detailed history, assessment/evaluation, scoring, and report including diagnosis (as appropriate) and recommendations for appropriate care.
(The above cost is based on 60-90 minute intake, and 10 hours total of evaluation, scoring, interpretation and report writing. Should the evaluation require time greater than 10 hours, each hour will be billed at a rate of $200 per hour.)
An Independent Educational Evaluation is completed as requested by the parent when there is disagreement with the findings during an IEP. A parent is able to request an IEE once a year, at the expense of the school district. Dr. Curtis is on the North County Coastal and North County Inland SELPA lists, as an IEE provider. A parent is also able to ask for any qualified individual to complete an IEE, regardless of placement on the official county lists.

Payment is due in full at the beginning of each session.
I accept cash and all major credit cards for payment.
Cancellations must be made 24 hours prior to the session.
My standard fee will be charged for missed appointments and cancellations made less than 24 hours.
